• You have been dealing with severe drug or alcohol use and it is impacting your relationships, work, or school. 
• You may be struggling with mental health symptoms and substance use and need specialized care to address both.
• You’ve tried outpatient programs, getting sober on your own, or going to aa and have found you need more support.
• You don’t have a great support system at home and feel like you need separation to start your recovery.
• You are motivated and ready to commit to a comprehensive treatment plan that focuses on your recovery.

RESTORE SCHOLARSHIP FORM

Located in Los Angeles, Restore Rehab Detox Treatment Center will cover the costs of lodging, food, and therapy services for those awarded the scholarship. This program requires clients to live full-time at the facility for the duration of the program, which can last from several weeks to several months depending on the patient’s needs. All patients undergo a comprehensive assessment in order to receive a program that is tailored to meet their particular needs. Restore does not cover medication copayments or transportation to and from the facility. All recipients of the scholarship will be treated the same as paying clients.

Substance use treatment refers to a variety of evidence based treatments designed to help individuals overcome reliance to drugs and/or alcohol through mental health therapy, medication-assisted treatment, behavioral therapies and group therapy. The goal of residential treatment is to identify physical and psychological aspects of substance abuse, helping individuals to learn coping strategies, find meaning, and work towards lasting recovery. 

Once you submit your scholarship form, we will share your responses to the admissions team at Restore Health & Wellness Center. They will contact you to set up an assessment to see if you are an appropriate candidate for their treatment program. From there, you will be contacted by Restore if you have been selected for their scholarship.
